Listed below are the names of personnel to be contacted in the event of an emergency or other situation that may require reporting under DOE 232.2A-Occurrence Reporting and Processing of Operating Information.

FOR 911 CALLS AND ALL ACCIDENTS (WITH OR WITHOUT INJURIES), PLEASE NOTIFY CASSANDRA HAYDEN AS SOON AS POSSIBLE.

If Cassandra is not available, please call the people on the list, in order, until you contact someone. During off hours, home phone numbers should be used first. DOE Order 232.2A states:

"Categorization of Reportable Occurrences shall be made as soon as practical and, in all cases, within two hours of identification."

IT IS FURTHER REQUIRED THAT THE DIVISION OFFICE MAKE ORAL NOTIFICATION TO DOE AS FOLLOWS

Emergencies: within 15 minutes

  • (An occurrence that results or could result in significant off site consequences.)

Unusual: within two hours of categorization

  • (An occurrence that has significant impact or potential impact on safety, environment, health, security or operations.)

A notification report is to be prepared and submitted as soon as practical, but in all cases, before the close of the next business day from the time of categorization (not to exceed 80 hours).
