I was not aware that 3 out of 3 units at NSLS had demonstrated this problem.  We had originally considered this issue rare due to the fact that the MAXv controller has been in the field for more than 5 years and this issue just now came to surface.  Given this recent window we have been auditing our assembly processes to see if we have a production issue that could be adding to this problem in addition to the firmware issue.  We are still pursuing this production possibility. 

I agree that providing the new firmware upgrade across the MAXv family is good.  We will offer the firmware upgrade as our normal warranty policy to those boards under the warranty period, and those boards that are outside the warranty period we will offer the firmware upgrade at no charge (shipping charges apply).

This seems to be a serious problem with an entire batch of MAXv's.  We've now seen the problem on 3 out of 3 units at X-17B2 at NSLS, and a number of other beamlines.  I think Pro-Dex should offer firmware upgrades even on out-of-warranty boards for this problem, since users may not become aware of the problem until after the warranty expires.
