This turned out to be a bug in StreamDevice that was reported last year (http://www.aps.anl.gov/epics/tech-talk/2015/msg01836.php) and subsequently fixed.

I upgraded StreamDevice to tag version stream_2_7_7 and now the problem has gone away.
