Also, in the CA reference manual documentation for ca_context_create(),
it says, "if preemptive callback mode is enabled the application is
no longer burdened with the necessity of periodically polling the CA
client library in order that it might take care of its background
activities."  I read that to mean calling ca_pend_event is unnecessary
when in preemptive mode.
