I used this many years ago and at the time found it flexible and easy to use.  I liked it for what I needed to do at the time.  I was pleased to see that it is still actively supported.

Yes I continue to send out copies and support the ORNL electronic network 
software.  Thanks for your interest in our Electronic Notebook software.
Information about our latest version along with an online demo can be found at:
http://www.csm.ornl.gov/~geist/java/applets/enote/

The software for the latest version (1.12) is freely available for download. 
And you are free to customize it to your particular needs.
The notebook consists of a Perl-5 CGI script and library, 
and some supporting GIF images. The computer you install the
software on will need to have Perl-5 and a web server installed (since the
notebook is web-based). Typically it is installed on a department web server,
since each installation can manage multiple notebooks. In this case,
Users of the notebook do not need to install anything - 
all they need is a web browser (even a smart phone).

We have it bundled up as either a  tar file, or as a ZIP file,
You can get them directly from:

   http://www.csm.ornl.gov/~geist/enote/zips/enote1_12.zip

The distribution includes installation instructions and 
describes options for customization to your particular needs.
Installed, the entire notebook package consumes less than 1 Mbyte
(most of this is the gifs).
