This is mostly a bugfix release. I added more details (and also fixes, thanks 
to Lewis Muir) to the docs and introduced reference counts to more 
consistently handle shared fellers. Details as usual at

Oh, I think I forgot to mention in the ChangeLog that I renamed 
fellerGetHeader to fellerGetPrefix; this was already changed in 0.3, actually.
