I checked in Help, About, Installation Details, Plug-Ins:
org.csstudio.platform.libs.epics.ui - does NOT show up
org.csstudio.ui.preferences - does show up

Plugin.xml looks exactly how you described!
Silly question: how to check the dependencies?

If some of these plugins should be included but don't show
up in the running product, check their dependencies.

If all fails, go back to the original product from which you
started, like KEK 3.0.1: When you run that, does it still
include the EPICS preference page, or is something fundamentally
wrong?

My CSS product is based upon the KEK 3.0.1. It has 3 features -
org.csstudio.kek.eclipse.feature, org.csstudio.kek.core.feature and
org.csstudio.kek.applications.feature. I have added alarms and archiving
to the applications feature. This epics prefs menu resides in the core
feature - org.csstudio.platform.libs.epics.ui, org.csstudio.ui.preferences
and org.csstudio.ui.help. So these three are in there already, nothing to
change. Still the problem exists.

I was creating my own CSS application in Eclipse RCP, added some features
- Alarms (Alarm Engine, Alarm Config Tool, Annunciator, JMS2RDB), dummy
Auth&authorization and the Archiving system (Archive Engine, Archive
Config Tool). Exported to a complete CSS application. Everything works
absolutely fine, but the 'Epics' menu in Edit->Preferences->CSS core has
disappeared! Of course, I can use the -pluginCustomization ini file, which
i do in any case. Just thought it doesn't look nice since it's not in the
Prefs anymore.

Any suggestions?
