It looks like you've tried to comment out stuff that you don't think you need, 
but you actually do need one more thing.  In particular you're not building 
the firstioc_registerRecordDeviceDriver.cpp file into your binary, which 
contains a static C++ constructor that registers all of the missing commands 
with the IOC shell.  You might be able to get away with not adding base.dbd to 
firstioc_DBD if you get all of the necessary menu*.dbd files, but you 
definitely need the above C++ source file in the firstioc_SRCS list.

Hopefully that fix will be enough to get you going.
