you don't have the current directory in your $PATH (and rightfully so).
Launch medm using a relative path specification, i.e. ./medm from inside
~/epics/extensions/bin/linux-x86 or an absolute path spec, i.e.
~/epics/extensions/bin/linux-x86/medm from anywhere. In the long run,
you'll probably want to expand your $PATH to contain the extensions bin
directory via

export PATH=$PATH:~/epics/extensions/bin/linux-x86

in your ~/.bashrc or the system-wide equivalent in /etc in order to
launch any EPICS extension from the command line by just typing its
name. While you're at it, do the same for the EPICS base bin directory.
