We have received new 6008-SV modules that failed.
The problem was identified as a firmware bug.
The problem appears to be with the prom located nearest to the VME
connector. I can not be sure which prom versions are bad. However I
suspect any V2.x version. We replaced the V2.x prom with a copy we made
of a V1.x prom and it worked just fine.
The symptoms I saw were queue overflow messages and dbior showed many
command errors and a maximum of about 10 block transfers per second.
This was on a system that does >40 block transfers per second.
