One of two things probably happened.

1) iocInit did not complete. Check this first by carefully looking at
   messages displayed on vxWorks console while iocInit is executing.

2) Something wrote to illegal part of memory including the LSET
   field of a record. This happens for one of two reasons:
  a) Bad record/device support. Common problem when developing new
      record/device support.
  b) The record/device support was compiled against a different version
       of epics than default.dctsdr. This happens if fields in record
      are changed between epics versions. Perform a gnumake clean
     and then rebuild local record/device/driver support.

If you dont have quick success try removing all record instances that
use locally written record/device/driver support and slowly add it
back until problem can be isolated to particular record/device support.
