We get "Events lost" frequently when someone runs X applications
from home via a dial-in line, and the speed of his link cannot
handle all of the traffic. Same can happen if the Unix host
is overloaded, or the network is saturated. Least likely is
an poorly designed database which simple overruns the IOC's
ability to get monitors onto the network.
That last scenario can happen if you have an interrupt service
routine or some other high priority operation suddenly take
too much CPU time (spin).
