EPICS env parameters

There are several places where the defaults for EPICS environment
parameters can be found. At first you find include/envDefs.h then you 
discover that this is overwritten by src/libCom/epicsEnvParams.h. All 
of this seems to be redundant (and inconsistent because not all of the 
parameters are in epicsEnvParams.h).

I would prefer that we set the global default for each project from 
an ASCII file and not by modifying source files - epicsEnvParams.h / 
envDefs.h.
