I updated entry 111. In summary, I'm not convinced that Ken's change will be
considered to be appropriate in all situations where CA is used. I will be
working the issue with Ken. A more complicated fix may be required.

This is only a problem for single threaded codes that did not follow our
original recommendation to call ca_poll regularly.

As the last entries there state, I committed the suggested patches but can't
test them because I don't have vxWorks 6.0.

I committed a fix, but can't test it because I don't have gcc 4.0 installed
here (it's not yet available from cygwin and my linux system isn't that
new).

Ok with me, but this presumes that the status of a decision regarding, and
or effort towards, a fix for 111 will not holdup the release.
