Configuration Control Work Permit Policy and Procedure
Policy
It is the responsibility of the APS users, as well as the APS personnel, to ensure that the beamline radiation protection systems are not compromised. Beamline hardware required for radiation protection of personnel at the APS is under the administrative control of the APS. These configuration controls are administered by the APS Floor Coordinators, and, prior to beginning any work on a component under configuration control, a Configuration Control Work Permit (Form UO-29) must be approved and posted by a Floor Coordinator. These yellow forms shall remain posted in the beamline cabinets at the end of the affected beamline for the duration of the work on the component.
[Note: Items under configuration control shall be tagged by a Floor Coordinator with a Configuration Control tag. In addition to the tagging, station labyrinths will be sealed with tamper evident seals.]
Procedure
Before any work is started
The following steps shall be completed before work is started on any component under configuration control.
- A person authorized by the group that manages the beamline or the APS person responsible for the work, shall complete the To Be Completed By Requester portion of the Configuration Control Work Permit. The scope of the work should be clearly defined in this section of the permit.
- The permit is submitted to a Floor Coordinator and the User Enable key(s) for the effected station(s) is (are) turned over to the Floor Coordinator. The requester is responsible for identifying to the Floor Coordinator potential hazards associated with their work. The Floor Coordinator will complete the second portion of the permit, identifying what additional approvals will be required to bring the beamline station(s) back on-line.
- The Floor Coordinator takes the effected station(s) off-line removing the APS Enable key(s).
- The Floor Coordinator locks the User Enable key(s) in the glass front cabinet at the end of the beamline.
- The key may be removed from the cabinet and issued to APS personnel for shielding or interlock validation. Such activities shall be noted on the Configuration Control Work Permit, and the key shall be returned to the cabinet at the completion of the testing.
- The Floor Coordinator covers the Configuration Control tags on the component with a Work in Progress tag or, if required for baking the component, the tag may be removed.
- The Floor Coordinator removes any tamper-evident seals.
- The Floor Coordinator updates the operations logs to show that station is off-line.
- The Floor Coordinator approves the work permit and posts it in the beamline cabinet at the end of the beamline.
The work on the component is completed.
Re-enabling a Station
- The requester notifies the Floor Coordinator that the work has been completed.
- The Floor Coordinator ensures that the required reviews, realignment, and revalidations have been completed and required approvals obtained.
- The Floor Coordinator restores the tags and seals.
- The work permit is removed from the beamline display box.
- As directed by the requestor, the beamline APS Enable may be restored.
- The User Enable key is returned to the beamline personnel.
- The Floor Coordinator completes the Restoration of Administrative Controls section of the permit.
The Floor Coordinator archives the permit and updates the operations logs.
