The Experimental Safety
Approval Form (ESAF)
The ESAF consists of two parts: the Experiment Authorization Form (EA) and the Experiment Hazard Control Plan (EHCP.)
The EA form lists general information such as the experiment PEN and ID numbers, experiment start date, On-Site Spokesperson, Safety Review and Approval for both the beamline and APS, brief material and equipment hazards listings, and final Safeguards Verification. This form gets posted in the beamline endcabinet.
The EHCP form gives a more detailed explanation of the experiment, including some information from the EA form. This form gets posted on the experimental station door where the experiment will be running. This form additionally lists:
- the materials to be used in the experiment, their quantity, and any hazards associated with the materials
- equipment and physical hazards with full experiment description
- hazard classes that apply
- equipment safety requirements summary
- beamline comments with risk class
- APS Experiment Review Board comments
- experiment personnel list and APS core training due dates
The ESAF form is generated in electronic version by the XSD and CAT staff. The details of how the
form is generated, and the experiment review process, are not of direct
concern to the FC. Nevertheless, there are several administrative
aspects that do directly involve the FC. They are discussed in the following procedure which should be read at this time: Instructions for Posting of Experiment Authorization Form/Experiment Hazard Control Plan
